A Closer Look at NHTD: What Is It and Who Is Eligible?

Nursing Home Transition and Diversion (NHTD) is a Medicaid waiver program that helps Medicaid-eligible seniors and individuals with physical disabilities. This home and community-based program provides comprehensive support to these individuals while they live in a community-based setting, instead of a nursing home or other institution.

This may entail the services of a registered nurse and other members of their healthcare team based on their unique choices, needs, strengths, and goals. With the NHTD, the participants are the primary decision-makers when it comes to their care and the level of support they receive.
